ABSTRACT

Objective:Analyze the influence of blood transfusion therapy under the guidance of thromboelastography on the blood potassium and coagulation indicators and prognosis of patients with post-traumatic hemorrhage.Methods:The clinical data of 73 patients with post-traumatic hemorrhage in Liyang People′s Hospital from March 2018 to February 2021 were retrospectively analyzed. Among them, TEG blood transfusion guidance group (group A) and conventional coagulation indicator guidance group (group B) were divided according to whether TEG test was performed. Repeated measurement analysis of variance and post LSD- t test were used to observe serum potassium, coagulation indexes and prognosis of 2 groups at each time point. Results:The amount of various drugs used in group A was significantly lower than that in group B ( P<0.05). Comparison of serum potassium levels between the two groups before transfusion: (3.94 ± 0.85) mmol/L vs. (3.98 ± 0.71) mmol/L; and on the first day after transfusion: (4.33 ± 0.48) mmol/L vs. (4.57 ± 0.73) mmol/L, there were statistically significant ( P<0.05); on day one. day two and day three after blood transfusion, the indexes of PT and APTT in group A were significantly lower than those in group B: PT: (14.30 ± 1.43) s vs. (16.25 ± 1.74) s, (14.41 ± 1.55) s vs. (16.27 ± 1.48) s, (14.73 ± 1.50) s vs. (16.30 ± 1.45) s; APTT: (32.3 ± 3.6) s vs. (36.7 ± 3.5) s, (32.6 ± 3.4) s vs. (36.8 ± 3.6) s, (32.2 ± 3.2) s vs. (36.3 ± 3.3) s; the above indexes of the two groups before treatment were significantly higher than those at each time point after treatment ( P<0.05). The total effective rate of hemostasis in group A was higher than that in group B: 97.37%(37/38) vs. 80.00%(28/35), P<0.05. Conclusions:Patients with post-traumatic hemorrhage can be recovered by blood transfusion under the guidance of thromboelastography, and the disorder of blood coagulation indicators can be restored, and at the same time, it has a better hemostatic effect.

ABSTRACT

INTRODUCTION@#Unmet psychosocial concerns are associated with emotional distress among cancer patients. This longitudinal study aimed to identify specific psychosocial concern profiles and trajectories of emotional distress, and examine their association among newly diagnosed adult cancer patients across the first year of diagnosis.@*METHODS@#Adult patients aged 21-64 years were screened to determine their eligibility for this study. Psychosocial concerns and psychological distress were measured using the Problem List and the Distress Thermometer, respectively (n = 221). Latent transition mixture analysis was used to determine specific psychosocial concern profiles and trajectories of emotional distress, and examine associations with adjustments made for demographic and medical variables.@*RESULTS@#Two classes of psychosocial concerns were identified: low (81%) and high (19%) concerns. Two trajectories of distress were identified: low stable (69%) and high stable (31%) trajectories. Patients in the high concerns class were significantly more likely to demonstrate the high stable trajectory of distress.@*CONCLUSION@#Our findings highlight the importance of concurrent assessment of multiple psychosocial concerns and screening of emotional distress throughout a cancer patient's treatment journey. Such assessments can effectively guide interventions to address individual concerns and alleviate emotional distress among newly diagnosed cancer patients.

ABSTRACT

INTRODUCTION@#Fear of cancer recurrence (FCR) among cancer survivors is a persistent and distressing psychosocial concern that affects recovery and quality of life. The prevalence of FCR in Singapore is unknown. This cross-sectional study was designed to examine FCR and identify factors associated with FCR in mixed-cancer survivors locally.@*METHODS@#Cancer survivors in remission (n = 404) were assessed for: FCR using the Fear of Cancer Recurrence Inventory (FCRI); emotional distress using the Hospital Anxiety and Depression Scale; and quality of life using the World Health Organization Quality of Life-BREF. Clinical and severe/pathological FCR was determined based on the severity scale of FCRI, known as FCRI-Short Form. Multivariate logistic regression was performed to examine factors associated with FCR.@*RESULTS@#The mean score on the FCRI was 59.5 ± 30.4. 43.6% of cancer survivors had clinical FCR and 32.1% had severe/pathological FCR. Younger age (odds ratio [OR] 0.952, 95% confidence interval [CI] 0.911-0.995, p < 0.05), higher educational status (OR 2.55, 95% CI 1.15-5.65, p < 0.05) and higher levels of emotional distress (OR 1.17, 95% CI 1.10-1.24, p < 0.001) were significantly associated with severe/pathological levels of FCR.@*CONCLUSION@#The present study is the first to determine levels of FCR among cancer survivors in Singapore. While the total FCR scores were similar to those of international studies, severe/pathological levels of FCR were found to be four times higher. These findings highlight a problem that is not widely recognised or acknowledged, but which deserves greater attention.

ABSTRACT

AIM: To explore the role of purinergic signaling mediated by ATP in the Alzheimer ’ s disease (AD)-related colon motility disorder and its related molecular mechanisms .METHODS:(1)Clinical trials:AD patients in our hospital were collected and studied .Radioimmunoassay was used for the determination of plasma motilin (MTL), cholecystokinin (CCK), vasoactive intestinal peptide (VIP) and nitric oxide (NO), and high-performance liquid chroma-tography ( HPLC) was applied to test the level of adenosine triphosphate ( ATP) .The patients were assessed by neuropsy-chology and scored accordingly .( 2 ) In animal experiments , AD mice received Morris water maze test , and the spatial learning and memory function were evaluated .The plasma levels of MTL , CCK, VIP and NO were examined by radioimmu-noassay , and the level of ATP was measured by HPLC .Choline acetyltransferase ( ChAT ) , VIP, nitric oxide synthase ( NOS) and ATP synthase were detected by immunohistochemistry .Western blot and immunohistochemistry were used to detect the expression of P2Y receptor.(3) In vitro, organ bath was applied to observe the effect of α,β-methylene ATP (α,β-MeATP), an agonist of P2Y receptor, on both spontaneous and electrically evoked contraction of colonic smooth muscle strip, and the technique of intracellular microelectrode was applied to observe the effect of α,β-MeATP on the membrane potential of colonic smooth muscle cells .RESULTS:Compared with control group , the levels of MTL and CCK were decreased (P<0.01), and the levels of NO and ATP were increased (P<0.05 or P<0.01), while the VIP level was not changed.Mini-Mental State Examination (MMSE) score was decreased (P<0.05), Alzheimer’s Disease Assess-ment Scale-Cognitive Subscale (ADAS-Cog) score, Neuropsychiatric Inventory (NPI) score and Alzheimer’s Disease Co-operative Study-Activities of Daily Living Scale ( ADCS-ADL ) were all increased as compared with control group ( P <0.01).The 4~6 d escape latency of APP/PS1 AD mice was significantly prolonged (P<0.05), and the space explora-tion ability distinctly reduced (P<0.05).In AD mice, the levels of MTL and CCK were decreased (P<0.01), and the levels of NO and ATP were increased (P<0.05 or P<0.01), while the VIP level was not changed .The protein expres-sion of colonic ATP synthase was significantly increased (P<0.05), but the expression of ChAT, VIP and NOS was not changed.The expression of P2Y receptor was increased (P<0.01).The results of in vitro experiment displayed that α,β-MeATP, from 20 μmol/L to 100 μmol/L, inhibited the spontaneous contraction of colonic smooth muscle strip in the nor-mal mice and AD mice ( P<0.05 or P<0.01 ) , and this inhibition was reversed by Na +channel inhibitor tetrodotoxin (TTX) (P<0.05 or P<0.01).In addition, the effect of α,β-MeATP at 100μmol/L on the AD mice was more obvious than that on the normal mice (P<0.05), and this inhibition was also antagonized by TTX (P<0.05 or P<0.01), pro-minent in AD group as compared with control group (P<0.05).In 10 Hz electrically evoked contraction of colonic smooth muscle strip,α,β-MeATP inhibited both the normal and AD mice (P<0.05 or P<0.01), while the inhibition was more obvious in the AD mice at the concentration of 40μmol/L or 100μmol/L (P<0.05 or P<0.01).CONCLUSION:AD patients and AD mice are accompanied by decreased MTL and CCK levels , and enhanced NO level , thus inducing colonic motor dysfunction along with AD .Meanwhile, ATP in plasma, purinergic neurons , and P2Y receptor expression are in-creased in the AD mice .Purinergic signaling mediated by ATP inhibits colonic smooth muscle strip contraction and further paralyzes the colonic movement function in AD .

[ ABSTRACT] AIM:To investigate the inhibitory effect of Am80 on neointima hyperplasia in carotid arteries after balloon injury and to observe the interaction between Krüppel-like factor 4 (KLF4) and retinoic acid receptorα(RARα). METHODS:Neointima hyperplasia in carotid arteries was observed by hemotoxylin and eosin staining.The expression of KLF4 and cyclin D1 was examined by immunostaining and Western blotting analysis.To detect the interaction between KLF4 and RARαin the vascular tissue, the injured arteries were harvested, and the protein extracts were prepared and subjected to co-immunoprecipitation assay.RESULTS:Compared with injured group, Am80 significantly reduced neointi-mal hyperplasia and the thickness ratio of intima to media.Am80 not only up-regulated KLF4 or RARαexpression in caro-tid arteries, but also increased the interaction between KLF4 and RARαat tissue levels.CONCLUSION:Am80 inhibits neointima hyperplasia in carotid arteries after balloon injury by promoting the interaction between KLF4 and RARα.

ABSTRACT

[ ABSTRACT] AIM:To investigate the antipyretic effect of patchouli oil on lipopolysaccharide ( LPS)-induced fe-ver in rabbits.METHODS:Male rabbits (n=42) were randomly divided into 7 groups according to their body weight and basal body temperature, including control group, model group, western medical positive group, traditional Chinese medical positive group, and high, middle and low doses (2%, 1%and 0.5%) of patchouli oil groups.Subsequently, except the controls, the rabbits were injected with LPS at a dose of 1 mL/kg (2 mg/L) through marginal ear vein to establish rabbit fever model and the rabbits in control group received the same volume of NS.The rabbits in control group and model group were injected with 0.5%Tween-80 0.5 h late, and the rabbits in the other groups were treated with correspoonding drugs. The effect of patchouli oil on the body temperature was observed, and the levels of interleukin-1β( IL-1β) and tumor nec-rosis factor-α(TNF-α) in the serum, and prostaglandin E2(PGE2) and cyclic adenosine monophosphate (cAMP) in the hypothalamus were measured by radioimmunoassay.RESULTS: The body temperature and the levels of IL-1β, TNF-α, cAMP and PGE2 in model group were significant higher than those in control group.Patchouli oil notably inhibited the body temperature in the febrile rabbits.From 1.5 h to 5.5 h after administration, the body temperatures were increased by (1.06 ±1.55), (1.62 ±1.36), (1.38 ±1.22), (0.98 ±0.98) and (0.48 ±0.95) ℃in high patchouli oil group, re-spectively.From 3.5 to 5.5 h after administration, the body temperatures were elevated by ( 1.47 ±0.73 ) , ( 1.15 ± 0.68) and (0.63 ±0.54) ℃ in middle patchouli oil group, respectively.A tendency of downregulation of the elevated body temperatures was observed at every time point after administration in low patchouli oil group.Patchouli oil significantly decreased the levels of TNF-αin the serum and cAMP content in the hypothalamus, and attenuated the elevated tendency of the IL-1βlevel in the serum and PGE2 level in the hypothalamus.CONCLUSION:Patchouli oil evidently has antipyretic effect on LPS-induced fever in the rabbits.The antipyretic mechanism might be related to the inhibition of TNF-αlevel in serum and cAMP content in the hypothalamus.

ABSTRACT

Ponseti clubfoot treatment has become more popular during the last decade because of its high initial correction rate. But the most common problem affecting the long-term successful outcome is relapse of the deformity. Non-compliance with Ponseti brace protocol is a major problem associated with relapse. Although more comfortable braces have been reported to improve the compliance, they all have the same design and no significant changes have been made to the protocols. After refinement in the Ponseti method and emphasizing the importance of brace to parents, the relapse rate has been markedly decreased. Nevertheless, there are patients who do not have any recurrence although they are not completely compliant with the brace treatment, whereas other patients have a recurrence even though they are strictly compliant with the brace treatment. The aim of this article is to review the relapse of clubfoot and the function of the brace and to develop an individualized brace protocol for each patient by analyzing the mechanism of the brace and the biomechanical properties of muscles, tendons, and ligaments.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the effect of rapid teeth movement through reducing resistance and distraction using cone-beam computed tomography (CBCT).</p><p><b>METHODS</b>The left or right side of the mandible of 10 beagles (5 males and 5 females) was randomly selected as the control side (for the conventional distraction with a force of about 85 g) and the other side as the experimental side (subjected to reducing resistance and distraction). CBCT images were taken at 5, 10, and 15 days and also after retaining for 10 and 90 days after distraction for 15 days. The distance of teeth transportation, degree of inclination, alveolar bone density of the compressive area and root resorption were evaluated based on the CBCT images and Ez3D2009 visualization software.</p><p><b>RESULTS</b>No significant difference was found between CBCT-based measurement and direct measurement of teeth transportation distance within the mouths (P>0.05). Teeth inclination on the experimental side was slightly greater than that on the control side (P<0.05). The root resorption on the experimental side was minimal, and the bone density increased gradually along with the teeth migration. The experimental side showed a comparable newly formed alveolar bone density and similar X-ray features of the compressive area with the control side.</p><p><b>CONCLUSIONS</b>CBCT three-dimensional imaging can resolve the problems of overlapping and deformation of the two-dimensional images and is especially useful for measurement of teeth inclination and alveolar bone density in the process of reducing resistance and distraction.</p>

ABSTRACT

ObjectiveTo valuate the clinical effect of maintence of arteriovenous fistula by Songling Xuemaikang capsule.Methods40 cases were obtained from patients with chronic renal failure had received arterio-venous fistula operation,24 cases were observed with the combination treatment and 16 cases were served as control group,Hemorheology and difference value of vein diameter before and after treatment,blood flow of native arteivenous fistula and primary using time were observed.ResultsThe improvement degree of high shear rate and low shear rate were significantly as compared with that in the control group (P =0.000,P =0.001 ),while the blood flow after fistula maturation and ifference value of vein diameter were better than that in the control group( P =0.0356),primary using time between two groups were no significant differences ( P =0.056).ConclusionSonglinxuemaikang capsule was an effective drug for prevention high blood caogulation state,increasing blood flow of native arteivenous fistula,and enlargment vein diameter after arterio-venous fistula operation,which had certain value in clinical application.

ABSTRACT

Objective To investigate the intervention methods of patients with high-risk and complicated carotid artery stenosis. Methods Twenty-five patients with high-risk and complicated carotid artery stenosis were treated with stenting During the procedure, they used the individualized therapeutic programs, including the implantation of temporary cardiac pacing, ev3 filter, combination of predilation and postdilation, guiding catheter (coaxial technique), staged angioplasty and stenting, and multiple stent implantations. Results The technical success rate of the 25 patients was 100%. The symptoms disappeared or improved in 21 patients (84%). The intraoperative dislodgement of emboli occurred in 1 patient. Transient ischemic attack occurred in 1 patient. No patients died. Conclusions Treatment with stenting in patients with high-risk and complicated carotid artery stenosis is safe. The preoperative evaluation of risks, the use of intraoperative individualized treatment programs, and the postoperative standardized management are the preconditions for ensuring efficacy.

ABSTRACT

Objective To investigate the histological changes of rapid tooth movement in dogs treated by resistance reduction and distraction osteogenesis, aiming to establish an animal model and further to reveal the remodeling mechanism of rapid tooth movement. Methods A total of 8 local hybrid dogs were selected as subjects for this study. The second pre-molar was extracted on both sides. The experimental side underwent alvelor surgery for resistance reduction and a home-made tooth-borne intraoral distraction device was installed for rapid tooth movement, while for the other side (control side) only tooth-borne intraoral distraction device was used for rapid tooth movement. The longest active force-delivery span was 2 weeks, followed by 6-week retention. The distance between the moved tooth and anchor unit was recorded weekly, and radiography was performed for each side before and after distraction. The surrounding tissues including periodontal ligament and alveolar bone were sectioned for histological analysis. Results The average distance of tooth movement was 3.55mm on the experimental side and 1.11mm on the control side. The rate of tooth movement was notably higher (P<0.01) and no significant apical root resorption was detected by X-ray on the experimental side. The active alvelor bone remodeling was found on the tension and pressure sides. However, there was no significant difference between the experimental side and the control side after the retention period. Conclusion The rate of orthodontic tooth movement can be accelerated through resistance reduction and periodontal distraction without any unfavorable effects but at minimal anchorage loss.

ABSTRACT

A method for the determination of decabrominated diphenyl ether(decaBDE) in sediment samples at trace level using dispersive liquid-liquid microextraction based on the solidification of floating organic drop (DLLME-SFO) and high performance liquid chromatography-ultraviolet detector (HPLC-UV) has developed.Based on the data of interactive orthogonal array design, the optimization experimental conditions were obtained with BP artificial neural network model: 1.00 mL methanol as dispersive solvent, 35.0 μL dodecanol as extractive solvent, 10.00% NaCl, pH 5, and extraction in 10 min.The extraction recovery (ER) was 62.22% at the extraction conditions.The proposed method exhibited a wide linear range(3.5-1400 ng/g) with R~2 =0.9921.The limit of detection (LOD) and the limit of quantification (LOQ) of this method were 2.3 pg/g(S/N =2) and 5.6 pg/g(S/N = 5), respectively.The recoveries of real samples at different spiking levels of decaBDE were 104.2%, 98.4% and 97.7%, respectively.Extraction, concentration and separation procedures for decaBDE from the sediment sample were carried out by one step, and hence, the process of DLLME-SFO for decaBDE was shortened.

Objective To evaluate the feasibility and efficacy of emergency carotid endarterectomy in acute stroke induced by internal carotid artery thrombosis.Methods Two patients with acute stroke with internal carotid artery thrombosis were treated with internal carotid artery incision,thrombous extraction,carotid endarterectomy and angioplasty.Results The cerebral ischemia symptoms of patients were obviously improved.One patient showed complete rehabilitation,another patient could take self-care.No thrombosis and restenosis in internal carotid artery were formed during one to nine month follow-up.Conclusions Emergency carotid endarterectomy in acute stroke with thrombosis occlusion in internal carotid artery is safe and feasible.

ABSTRACT

Objective To sum up the clinical experience of operative management of tibia plateau fracture in senile patients. Methods 35 cases of senile tibia plateau fracture were treated with different ways of internal fixation, including screw, cannulated compression screw, L shaped plate, and T shaped plate. Results All the 35 cases were followed up and the time ranged from 6 months to 2 years. X ray films showed that the anatomical reduction was reached and the fractures healed in 33 cases. The final results were rated according to Merchant criteria: excellent in 22 cases, good in 9 cases, fair in 2 cases and poor in 2 cases. The excellent and good rates were 85.7%. Conclusions Operation on senile patients with tibia plateau fracture should be performed early so as to reconstruct the articular surface. Rigid fixation and bone transplantation are impartment means to avoid plateau collapse, and early functional exercise can reduce postoperative complications.

ABSTRACT

Objective To retrospectively review the experience and early clinical result of carotid endarterectomy (CEA) for the treatment of symptomatic carotid artery stenosis. Method CEA was performed on 82 consecutive patients (66 males, 16 females), age ranged from 48 to 84 years (mean 68.6 years). Diagnosis was established by carotid artery Doppler ultrasonography and DSA in all patients. General anesthesia was used in 39 cases, regional anesthesia was used in 43 cases. Temporary intraluminal shunts were used in 56 patients. Results There was no surgical mortality. Postoperatively 76 patients were followed up from 4 months to 18 months. Symptoms significantly improved in 57 cases, mild improvement was achieved in 14 cases, severe postoperative complication occurred in 3 patients, including cerebral ischemia in 2 cases, intracarotid thrombosis in one. Conclusion CEA is an effective therapy for the treatment of symptomatic carotid artery stenosis.

ABSTRACT

Objective To determine an optimal method of in vivo endothelialization when homograft valve (HV) leaflets are treated with different methods. Methods Twenty five fresh HV leaflets were divided into five groups randomly. The leaflets of GroupⅠ were fresh HV ones without any treatment; GroupⅡwere cryopreserved in liquid nitrogen for 1 month; Group Ⅲ was treated by DOA; GroupⅣ was treated by 25 g?L -1 glutaraldehyde; Group Ⅴ was first treated by hypotonic/hypertonic, then with an enzyme-based solution. Each HV leaflet was trimmed to 2 mm one which was cultured with lymphocyte from the recipient. In order to compare the antigenicity of HV leaflets, 3H-TdR incorporation was observed after tissue-cell coculture and stimulous index (SI) was calculated. The HV leaflets of different groups were implanted into the sheep's femorofemoropopliteal for three months; then the following procedures were done. Immunohistochemistry of Ⅷ factor correlative antigen was used to identify the cells property; observation of growth condition of EC and superficial morphosis of HV leaflets was made with scanning election microscope (SEM).
